REPORTS TO: Director of Operations
SUPERVISES: Direct Support Professional
GENERAL RESPONSIBILITIES: Work as Direct Support Professional, as well as supervise, support and train DSP in ensuring quality of services provided and the health and safety of the individual served.
SPECIFIC RESPONSIBILITIES:
- Collect and disseminate forms and agency information as delegated by supervisor.
- Train and monitor DSP
- Develop and manage staffing schedule in accordance with Plan of Care.
- Responsible for ensuring that the individual’s home is safe, clean and in good repair.
- Collect and manage individual’s financial information as required.
- Monitor individual’s health care needs.
- Monitor proper diet, meal planning and shopping.
- Ensure that monthly safety drills are conducted and documented.
- Must report any suspected abuse, neglect or exploitation immediately to supervisor.
- Maintain documentation, certification and training as required to comply with federal, state and program requirements standards.
- Perform other duties as assigned.
- Comply with all standards to assure the health and safety of all staff and clients we serve.
- Review of goals and preparation of goal sheet.
- Planning and follow-through of doctor appointments.
- Planning living arrangements for clients.
- Moving clients into their new homes as required.
- Assist in crisis situations.
- Coordinate staff coverage.
- On call 24/7
- Must be flexible to cover a shift or fill in when the need arises.
QUALIFICATIONS:
- Valid driver’s license
- Experience working with people with developmental disabilities preferred
- High school diploma or equivalent (GED)
- Must be at least eighteen (18) years of age
- Must pass background check(s) required by law
- Delegated nursing certificate
- CPR/First aid certification
- Clear driving record (drivers abstract)
- Eight (8) hours DD orientation
